How Deductibles, Copays and Coinsurance Work in Michigan (2027 Examples)

Four numbers decide what you pay when you use health insurance. Once you see them work together in an example, plan comparisons get much easier.

The four terms

  • Deductible: what you pay for covered care before the plan starts sharing costs. Preventive care is free before the deductible on ACA plans.
  • Copay: a flat fee for a service, like $40 for a doctor visit or $15 for a generic. Some plans apply copays before the deductible; others only after.
  • Coinsurance: your percentage share after the deductible, like 20% of the allowed amount.
  • Out-of-pocket maximum: the most you pay in a year for covered in-network care. After that, the plan pays 100%. For 2027 ACA plans the cap is $12,000 for one person and $24,000 for a family.

Your premium, the monthly bill, doesn't count toward any of these.

Example: a $20,000 surgery

Plan: $3,000 deductible, 20% coinsurance, $7,500 out-of-pocket max.

  1. You pay the first $3,000 (deductible).
  2. Of the remaining $17,000, you pay 20%, which would be $3,400.
  3. $3,000 + $3,400 = $6,400, under the $7,500 max, so you pay $6,400 and the plan pays $13,600.

If later that year you have another $10,000 bill, you'd only pay $1,100 more before hitting the $7,500 maximum. After that, covered care costs $0 for the rest of the year.

Example: a normal year with copays

Plan: $5,000 deductible, but $30 primary care and $15 generic copays apply before the deductible. Four doctor visits and a monthly generic cost you 4 × $30 + 12 × $15 = $300 for the year, and the deductible never comes into play. On a plan without pre-deductible copays, the same care might cost $800 or more.

Family plans: embedded deductibles

Most family plans have an embedded individual deductible and out-of-pocket max inside the family amount, so one sick family member doesn't have to meet the whole family deductible alone. ACA rules require each person's out-of-pocket costs to stop at the individual cap ($12,000 in 2027).

What resets

ACA plans reset on January 1. Private plans may reset on your policy anniversary. If you switch plans mid-year, you usually start over at $0. See how to switch plans.

In Michigan, open enrollment for 2027 coverage runs on HealthCare.gov from November 1, 2026 to January 15, 2027. Pick a plan by December 15, 2026 if you want coverage to start January 1, 2027; plans picked after that start later.

Frequently asked questions

What's the difference between a copay and coinsurance?

A copay is a flat dollar amount per service. Coinsurance is a percentage of the allowed cost, usually after you've met the deductible.

Does my premium count toward my deductible?

No. Premiums are what you pay to keep the plan. Only your share of covered care counts toward the deductible and out-of-pocket maximum.

What is the maximum out-of-pocket for 2027?

$12,000 for an individual and $24,000 for a family on ACA-compliant plans.
