How Prescription Drug Coverage Works in Michigan (2027 Guide)
Two plans with the same premium can charge wildly different amounts for the same drug. The difference is in the plan's formulary, the list of covered drugs and the tier each one sits on.
Drug tiers
| Tier | Usually includes | Typical cost |
|---|---|---|
| 1 | Preferred generics | Low copay, sometimes $0–$15 |
| 2 | Other generics, preferred brands | Moderate copay |
| 3 | Non-preferred brands | Higher copay or coinsurance |
| 4–5 | Specialty drugs | Coinsurance, often 20–50% |
Tier names and counts vary by plan. Always check the specific formulary.
Deductible or copay?
Some plans give you drug copays right away. Others make you pay the full negotiated price until the deductible is met, which can mean hundreds of dollars a month for a brand-name drug in January. This one detail often matters more than the premium.
Rules that can slow you down
- Prior authorization: your doctor has to get approval first.
- Step therapy: you have to try a cheaper drug before the plan covers the one prescribed.
- Quantity limits: caps on how much you can fill at once.
If a drug isn't on the formulary, ACA plans have an exceptions process: your doctor can request coverage, with a faster decision for urgent cases. Denied? See how to appeal.
How to pay less
- Search every prescription in each plan's formulary before you enroll.
- Ask for a 90-day supply through mail order.
- Ask your doctor about a generic or preferred alternative.
- Check manufacturer savings programs. Many brand drugs have copay cards for commercially insured patients.
- Compare cash prices; sometimes a discount price is lower than your copay. It may not count toward your deductible.
Private and Medicare plans
Outside the marketplace, private shared-network plans are sold in Michigan on leased PPO networks (Cigna, Aetna and PHCS PPO). They're medically underwritten, so they ask health questions and price on age and health instead of income, but you can enroll any month and coverage starts on the 1st. Private plans have their own formularies and may limit brand or specialty drugs, so check before you switch.
For Medicare Part D, the annual out-of-pocket cap on covered drugs is $2,400 in 2027.
In Michigan, open enrollment for 2027 coverage runs on HealthCare.gov from November 1, 2026 to January 15, 2027. Pick a plan by December 15, 2026 if you want coverage to start January 1, 2027; plans picked after that start later.
