15 Questions to Ask About Health Insurance in Michigan Before You Enroll (2027)
The questions you ask before you enroll decide whether you're surprised later. Here are the ones worth asking, grouped by topic, with what a good answer sounds like. Ask for everything in writing: a Summary of Benefits and Coverage (SBC) for ACA plans, or the full plan brochure and policy for private plans.
About the network
- Is this an HMO, EPO, PPO or POS? This tells you whether out-of-network care is covered and whether you need referrals. See HMO vs. PPO.
- Are my specific doctors and hospital in network? Get the network name, then call the office yourself.
- What happens if I'm traveling or my kid is at college out of state? Many local HMOs only cover emergencies outside the area.
About cost
- What's the deductible, and does it apply to office visits and generic drugs?
- What's the out-of-pocket maximum? For 2027 ACA plans it can be up to $12,000 for one person.
- Is the premium locked for the year? ACA plans are set for the calendar year; private plans may change at renewal or with age.
- Am I eligible for a premium tax credit or cost-sharing reductions? Cost-sharing reductions only come with silver plans.
About coverage
- Are my prescriptions on the formulary, and at what tier?
- Is there a waiting period or pre-existing condition exclusion? ACA plans can't have them. Private underwritten plans can.
- Are maternity, mental health and physical therapy covered, with any visit limits?
- Which services need prior authorization?
About the company and the person selling it
- Is this major medical insurance, or a fixed-indemnity, discount, or health-sharing plan? Get a straight answer.
- Are you licensed in Michigan, and which carriers do you represent? You can verify any agent's license with Michigan Department of Insurance and Financial Services (DIFS).
- How do I file an appeal if a claim is denied? See how to appeal a denial.
- When can I cancel or switch? In Michigan, open enrollment for 2027 coverage runs on HealthCare.gov from November 1, 2026 to January 15, 2027. Pick a plan by December 15, 2026 if you want coverage to start January 1, 2027; plans picked after that start later.
Red flags in the answers
- "Don't worry about the network, everyone takes it."
- "You don't need to answer the health questions."
- Refusing to send documents before you pay.
We'll answer every one of these in writing. Check your options and ask away.
Frequently asked questions
What should I ask before buying health insurance?
Ask about the network type and whether your doctors are in it, the deductible and out-of-pocket maximum, your prescriptions, exclusions or waiting periods, and whether the plan is real major medical coverage.
How can I check if an insurance agent is licensed?
Look them up with Michigan Department of Insurance and Financial Services (DIFS) or through the NAIC's national producer lookup.
What is a Summary of Benefits and Coverage?
A standardized document ACA plans must provide that shows deductibles, copays, coverage examples and exclusions in the same format, so plans are easier to compare.
