Do I Have to Disclose My Medical History When Applying for Insurance in Michigan? (2027)

It depends entirely on the type of plan. On ACA plans, insurers aren't allowed to ask. On medically underwritten private plans, they ask, and you're legally required to answer truthfully.

ACA plans: no health questions

Marketplace plans on HealthCare.gov, ACA plans sold off-exchange, and most employer plans can't ask about your health or charge more for it. The only health-related question is tobacco use, which can raise ACA premiums by up to 50% in most states. Pre-existing conditions are covered from day one.

Private underwritten plans: health questions required

Outside the marketplace, private shared-network plans are sold in Michigan on leased PPO networks (Cigna, Aetna and PHCS PPO). They're medically underwritten, so they ask health questions and price on age and health instead of income, but you can enroll any month and coverage starts on the 1st. The application asks about diagnoses, treatment, prescriptions and sometimes height and weight, usually over a set look-back period such as the last 2, 5 or 10 years. Read the wording carefully:

  • "Diagnosed with" is narrower than "diagnosed, treated, or consulted a doctor about."
  • A procedure or a prescription counts as treatment.
  • Conditions listed as examples usually aren't the full list.

What happens if you leave something out

This is where people get hurt. Underwritten plans check prescription databases and medical records, sometimes at application and often when a big claim comes in. If they find a material misstatement, they can rescind the policy (cancel it back to the start), deny claims, and refund premiums, leaving you with the bills. An agent who tells you to skip questions isn't doing you a favor.

How to handle it the right way

  1. Gather dates of diagnoses, treatments and medications before you apply.
  2. If you're unsure whether something counts, ask. A good agent will tell you how a carrier reads the question.
  3. If a condition will likely get you declined, apply for an ACA plan instead. That's what guaranteed issue is for.
  4. Keep a copy of your completed application.

Life events and enrollment timing

Because ACA plans have enrollment windows, people with conditions should plan around them. In Michigan, open enrollment for 2027 coverage runs on HealthCare.gov from November 1, 2026 to January 15, 2027. Pick a plan by December 15, 2026 if you want coverage to start January 1, 2027; plans picked after that start later.
