What Does Health Insurance Cover in Michigan? (2027 Guide)
It depends on what kind of plan you have. ACA-compliant plans have to cover a standard list. Private underwritten plans, short-term plans and supplements set their own rules. Here's the baseline, and how to check any plan.
What ACA plans must cover
Every marketplace plan, and every ACA-compliant plan sold off the marketplace, covers these 10 essential health benefits:
- Doctor visits and outpatient care
- Emergency services
- Hospitalization and surgery
- Pregnancy, maternity and newborn care
- Mental health and substance use treatment
- Prescription drugs
- Rehabilitative and habilitative services (like physical therapy)
- Lab tests
- Preventive and wellness care, and chronic disease management
- Pediatric services, including children's dental and vision
ACA plans also can't exclude pre-existing conditions or put annual or lifetime dollar caps on these benefits.
What's free
Recommended preventive care, such as annual checkups, many screenings, vaccines and well-child visits, costs $0 in network, even before the deductible.
What's usually not covered
- Adult dental and vision (sold separately)
- Cosmetic procedures
- Long-term custodial care
- Weight-loss drugs, on many plans (GLP-1 coverage varies widely)
- Experimental treatment
- Care outside the network on HMO and EPO plans
Private and non-ACA plans
Outside the marketplace, private shared-network plans are sold in Michigan on leased PPO networks (Cigna, Aetna and PHCS PPO). They're medically underwritten, so they ask health questions and price on age and health instead of income, but you can enroll any month and coverage starts on the 1st. These plans can exclude pre-existing conditions, limit maternity or mental health benefits, or cap certain services, so read the exclusions section before you buy. Fixed-indemnity and discount plans aren't major medical at all.
How to check any plan
- Read the Summary of Benefits and Coverage (ACA plans) or the plan brochure and policy (private plans).
- Look for the "services your plan generally does not cover" list.
- Check the drug formulary for each prescription. See how prescription coverage works.
- Ask about prior authorization for anything planned.
Mental health benefits have their own rules. See what's included in mental health coverage.
In Michigan, open enrollment for 2027 coverage runs on HealthCare.gov from November 1, 2026 to January 15, 2027. Pick a plan by December 15, 2026 if you want coverage to start January 1, 2027; plans picked after that start later. Check your options and we'll show you exactly what each plan in your ZIP covers.
